Fill out the form completely by either (1) completing the PDF form and then saving the form; or, (2) printing the form and completing by hand.
Once complete, email the completed and signed form to: petersburgkyfd@gmail.com
Schedule a time to meet at the Fire Department for a walk through and an interview.
Once a department officer has recommended your approval, the Petersburg Fire District Board must approve your membership.
